Nearly 400 restaurants citywide were at risk of being shut down this year after inspectors uncovered a host of health violations, ranging from improper hand-washing to roach infestations.

An initial report made to the Coast Guard on Nov. 16 said a pipeline rupture near the mouth of the Mississippi River released a maximum of 1.1 million gallons into the Gulf of Mexico.
